I understand that my<br \/>therapist is not qualified to perform spinal or skeletal adjustments, diagnose, prescribe, or treat physical<br \/>or mental illness.<br \/>_____<br \/>I affirm that I have notified my therapist of all known medical conditions and injuries. I agree to inform<br \/>the therapist of any changes in my health and medical condition. I understand that there shall be no<br \/>liability on the therapist\u2019s part should I forget to do so.<br \/>_____<br \/>I understand that bodywork is entirely therapeutic and non-sexual in nature.<br \/>_____<br \/>By placing an order, I hereby waive and release my therapist from any and all liability, past, present,<br \/>and future relating to massage therapy and bodywork.<br \/>_____<br \/>I understand that if I cancel an appointment less than 4 hours before the scheduled time\u00a0an appointment, I am subject to a fee equal to 50% from the cost of the missed appointment. If the<br \/>appointment was booked under a gift certificate, it will be voided in lieu of the fee of 50% from the cost of the missed appointment.
